I consent to my child participating in all activities involved in the programme.
I approve of the above application form and in doing so, agree that CAMP North Lancs, its Directors, Leaders, Staff and Instructors shall be released from and not incur any responsibility or liability for any accident or injury to the applicant or for any damage to or loss of the property of the chid.
I agree to the booking terms and conditions for CAMP North Lancs.
I can confirm that all information supplied is correct and any changes to the above application are to be notified to CAMP North Lancsat the earliest opportunity. |
